Dear Sir/Madam:

Greetings of peace!

This is to respectfully authorize MR. REYJEZUR CRUZ PIOCO to claim and/or process
the necessary documents for the Persons with Disabilities (PWD) Identification Card
of my daughter, MS. ROCHE FLUME B. BENDANILLO.

My daughter is a cancer patient who is currently undergoing Chemotherapy treatment

at Northern Mindanao Medical Center (NMMC) in Cagayan de Oro City. She could not
possibly process this herself because of the condition she is in as of the moment. She is
also accompanied now by her father, Mr. Felix G. Bendanillo, and brother, Mr. Jestoni
Bendanillo, who were the ones that processed her application for a PWD ID earlier this
month. I, on the other hand, could not be able to process/claim the ID myself because I
also have a physical condition that limits my movement since I suffered from a mild
stroke a while back.

With this, I am authorizing MR. PIOCO to claim the said ID for my daughter, MS.
BENDANILLO. Attached herewith are photocopies of our valid IDs and medical records
for your perusal.

Your assistance on this matter is highly appreciated. Thank you very much and God
