AFFIDAVIT OF BEING SINGLE

I, MARDEN R. DALAGUIADO, of legal age, Filipino, single and a
resident of Adtuyon, Pangantucan, Bukidnon, after having been duly sworn
to in accordance with law do hereby depose and say:

1. That I am applying in the Military Service as trainee in the Philippine

Army;

2. That I was born on March 03, 1998 and one of the children of spouses
Candido Dalaguiado and Maria Victoria Dalaguiado;

3. That I am SINGLE, as I have never contracted any marriage with

another person up to the present;

4. That I am executing this affidavit to attest to the truth of the above-

statement for whatever legal purposes for this affidavit it may serve.
