MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement, made and entered into by and between:

The SANGGUNIANG PANGLUNGSUD MUNICPAL BUILDING, with office address at

Purok 2, Liberad, Quezon, Bukidnon 8715, herein represented by MAYOR LORENZO POLING,
Municipal Mayor and hereinafter to as the “Institution”.

-and-

The PHILIPPINE COLLEGE FOUNDATION – Bachelor of Science in Office

Administration Department, a duly registered educational institution recognized by the Commission on
Higher Education with postal address at Purok 6, Poblacion, Valencia City, Bukidnon herein represented
by the Dean of the School of Business and Management, George C. Patron, MBA, of legal age, Filipino,
Married and presently residing at Lurugan, Mt. Nebo, Valencia City, Bukidnon and hereinafter referred as
the “College”.

CRISTEL MAE ERMAC, with residence and postal address at PUROK-19, POBLACION,
VALENCIA CITY, BUKIDNON hereinafter referred to as the “Student”.

WITNESSETH:
The BS in Office Administration Department of the “College” is committed to provide the
“Student” a comprehensive learning in the classroom and on the job training as well;

That to achieve this goal the “College” requires the “Student” a practicum of 300 hours on the job
the training as part of the BSOA Curriculum;

That as a partner in equipping and preparing the “Student” for quality service to the prospective
employer, country and mankind, the “Institution” makes available to the “Student” the requisite
practicum;

WHEREFORE, THE PARTIES HERETO AGREE”

1. The “College” subject to the approval of the “Institution” shall endorse the qualified “Student” for the
Practicum;

2. The “Student” shall comply with the 300 hours of On the Job Training requirement;

3. The “Institution” through the Practicum-in-charge shall provide orientation on work values, behavior
and discipline;

4. During the period of the Practicum there shall be no employer-employee relationship between the
“Institution” and the “Student”;

5. The period of the Practicum cannot be considered as Probationary Period and therefore the
“Institution” is not obliged to employ or hire the “Student” after the completion of the Practicum.
