Thank you for being our valued customer under our Group Policy, Family Health Floater, since
03/07/2022

As per our latest annual product performance review as per regulatory requirement we find that this product Family Health Floater
(under Group policy) is no longer financially viable and requires a course correction such as price increase.However, being an esteemed
customer as a special offer we are pleased to inform you that we will be renewing your policy with same product features and
pricing with full continuity benefit this year ie. you will continue to pay the same premium applicable as per your old product Family
Health Floater when you renew your policy in 2021-22 and 2022-23 . However,starting this renewal we will be migrating to alternative
product Group Health Policy instead of FHF (in Group Policy) and there is not going to be any likely impact on your premium and
product features.

Your policy is due for renewal on 02/07/2023 & the renewal premium due is Rs.67000/-(Inclusive Of Goods and Services tax).
Continuous renewal of policy is important as otherwise waiting periods would start afresh.
In case your present address is different from the address captured in your policy, request you to call us on the below number & share the
correct address, for the same to be captured in the policy. This will ensure correct receipt of policy at your end.
Medicare TPA has been acquired by Medi Assist Insurance TPA Private Limited, who will be servicing your cashless claims, going
forward. Your existing TPA card will continue to be valid and there is no change in the contact details shared there in.
