ALL foreign citizens traveling to Mexico MUST fill out an Official Entry Immigration Form
(FMM) prior to arrival to Mexico.

Step-by-step Guide for Filling out

Custom and Immigration Forms

• Be sure to have a pen with you while traveling to be able to fill out the forms.
• These forms will likely be handed out to you on your flight to Mexico or by an agent upon arrival.
• Make sure you fill out the “top half ” and the “bottom half” of the IMMIGRATION form.

I. TOP HALF: FILL OUT WHITE PART ONLY

1. First name
2. Last name
3. Nationality
4. Date Of Birth (DAY/MONTH/YEAR)
5. Gender (Male or female)
6. Passport number (found in the top right-hand corner of your passport)
7. Country of Residence (where you live permanently)
8. Blank
9. Purpose of trip: TOURISM!!!
10. Entering By: Air
11. Airline Code (If you don’t know the airline code just write the full name of the airline)
12. Flight number of your flight you are on (can be found on a boarding pass)
13. The name of the city you are flying into (most likely Mexico City)
14. Hotel name & or address

HOTEL ADDRESS: 15 de Mayo 4502, Zona Cementos Atoyac, 72023 Puebla, Pue., Mexico

II. SECOND PART (Bottom) - FILL OUT WHITE PART ONLY

1. First name
2. Last name
3. Nationality
4. Date of birth (DAY/MONTH/YEAR)
5. Male or female
6. Passport number
7. Tourism
8. Air (same as above)
9. Airline code or airline (same as above)
10. Flight number (check boarding pass)

• At the immigration desk the immigration agent will tear the small part of the form off and hand it back to you
with your passport.
• This piece of paper is your tourist visa.
• Take a picture of it immediately with your phone for back up and keep the original copy in a safe place (e.g. with
your passport)
• When you depart Mexico, you will need this form to leave. If you lose this little form, it will be a huge hassle.
Not only will you have to pay a fine, but you could potentially miss your flight depending on how much time you
have before your flight leaves and how busy the immigration office is.

• Asegúrese de tener un bolígrafo con usted mientras viaja para poder completar los formularios.
• Es probable que estos formularios le sean entregados en su vuelo a México o por un agente a su llegada.
• Asegúrese de completar la “mitad superior” y la “mitad inferior” del formulario de INMIGRACIÓN.

I. MEDIO SUPERIOR: LLENE LA PARTE BLANCA SOLAMENTE

1. Nombre
2. Apellido
3. Nacionalidad
4. Fecha de nacimiento (DÍA / MES / AÑO)
5. Género (hombre o mujer)
6. Número de pasaporte (que se encuentra en la esquina superior derecha de su pasaporte)
7. País de residencia (donde vives permanentemente)
8. Número de tarjeta o forma migratoria (se encuentra en el Formulario de inmigración debajo del código de barras)
9. Propósito del viaje: Turismo !!!
10. Blanco
11. Via de Internación: Aire
12. Código de la aerolínea (Si no conoce el código de la aerolínea, simplemente escriba el nombre completo de la
aerolínea)
13. Número de vuelo de su vuelo en el que se encuentra (se puede encontrar en una tarjeta de embarque)
14. El nombre de la ciudad a la que estás volando (probablemente Ciudad de México)
15. Nombre y dirección del hotel.

II. SEGUNDA PARTE (Parte inferior) - LLENE LA PARTE BLANCA SOLAMENTE

1. Nombre
2. Apellido
3. Nacionalidad
4. Fecha de nacimiento (DÍA / MES / AÑO)
5. Hombre o mujer
6. Número de pasaporte
7. Turismo
8. Aire (igual que arriba)
9. Código de aerolínea o aerolínea (igual que arriba)
10. Número de vuelo (cheque tarjeta de embarque)

• En el mostrador de inmigración, el agente de inmigración arrancará la pequeña parte del formulario y se lo

devolverá con su pasaporte.
• Este pedazo de papel es su visa de turista.
• Tome una foto de ella inmediatamente con su teléfono para hacer una copia de seguridad y guarde la copia
original en un lugar seguro (por ejemplo, con su pasaporte)
• Cuando salga de México, necesitará este formulario para irse. Si pierdes esta pequeña forma, será una gran
molestia. No solo tendrá que pagar una multa, sino que podría perder su vuelo dependiendo de la cantidad de
tiempo que tenga antes de que salga su vuelo y de lo ocupada que está la oficina de inmigración.
