AFFIDAVIT OF LEGITIMATION

We, Spouses JOSEPITO L. CAGAMPANG and JUDITH C. CAGAMPANG, of

legal age and residents of Awasian, Tandag City, Surigao del Sur, state under oath,
THAT:

1. We are the biological parents of JOHN VINCENT C. CAGAMPANG, born on

November 1, 2003;

2. John Vincent was born before our marriage, as we have been living together
as common law partners since year 2002. Hence, he was considered illegitimate child;

3. On October 28, 2009, we married each other at the Office of the Municipal
Mayor, Tandag City, Surigao del Sur;

4. Having been legally married, John Vincent has acquired legitimate status by
virtue of law;

5. We are executing this affidavit to affirm the legitimate status of our child,
named above, and modify the existing records and status in this act of legitimation.
