OCCUPANCY PERMIT: BASIC REQUIREMENTS

1.    Duly accomplished Certificate of Completion, signed by owner/ applicant, signed and sealed by respective architect/
engineers, Forms are available at the Office of the Building Official.
2.    Photocopy of all Approved Permit Application Forms ( Back & front)
3.    Copy of Approved Construction Plans/Blueprints
4.    Construction costs with Bill of Materials
5.    As-built Construction Plans (A3-size), duly signed by owner/ applicant, duly signed/ sealed by respective professionals 10,000.00
6.    Photocopy of updated PTR/PRC and Accredited Professional Organization ID
7.    Photocopy of valid ID of owner (preferably government-issued ID)
8.    Notarized Authorization Letter to transact in behalf of the owner
9.    Fire Safety Inspection Certificate (FSIC) from Makati Fire Department 10,000.00
10.    Photocopy of Construction Business Permit (Contractor’s Tax) or Certification for Completion (for non-Contractor) from BPLO

11.  Logbook with Daily Construction Works/Activities


12.  Pictures of site/Project showing substantial completion ,Front view, Rear view, Left Side, Right Side, Interior
13.  Contract of lease ( if property is not owned)
14.  Notarized Affidavit of Change Engineers/ Professionals
15.  PDF Copy of Approved Permit Forms and Construction Plans; Calculations and Certifications in CD/ DVD/ USB
16.  Certificate of Completion for Building/improvement
17.  Certificate of Completion for Electrical (by Professional Electrical Engineer) 5,000.00
18.  Certificate of Completion for Sanitary/Plumbing 5,000.00
19.  Certificate of Completion for Mechanical (by Professional Mechanical Engineer) 5,000.00
NOTE: The Structural Engineer/Professional Electrical Engineer who will sign and seal Plans and Calculations should
be the same
