Dear Mr. Walter Werner,

This refers to the “Promissory Note” you executed on ____________________ by which you
promised to pay the amount representing the hospital bill for patient ROJERINE KATE
CABALLAS the amount of Thirty One Thousand Pesos (Php 31,000.00) and professional fees
amounting to Fifteen Thousand Nine Hundred Two and 2/100 (Php 15, 902.02) with the total
amount of FORTY SEVEN THOUSAND FOUR HUNDRED NINETY EIGHT AND 23/100
(Php 47, 489.23) to Binakayan Hospital and Medical Center, payable in installment within 6
months.

In this regard, and in order to spare yourself from any inconvenience and humiliation of court
litigation, we demand from you the full settlement of your indebtedness to our client, Binakayan
Hospital and Medical Center specifically to Dra. Rosela Ferrer, by paying the total amount of
Twenty-Five Thousand Pesos (Php 25,000.00, Philippine Currency, plus legal interest of 6% per
month, commence from your actual default on May 13, 2020, within 15 days from receipt
hereof.

Consider this as our last and final demand to settle your obligation to Dra. Ferrer, otherwise,
upon instructions we will be constrained to initiate appropriate criminal and civil cases against
you in the Court of Law to protect the interests of our client.

We recommend you to act promptly to this final demand. Should you wish to clarify a matter,
please feel free to contact us at the cellphone numbers above stated or visit our client at anytime
of the day most convenient to you.

WE RESERVE ALL RIGHTS AND WAIVE NONE.
