II. REASON/S FOR NEGATIVE SLIPPAGE
1.0 Primary reason for slippage or project delay was the weather condition during the month
August. Heavy rains and typhoons causes work stoppage and flood water on excavated areas
consumed time for draining and pumping out.
2.0 Problems encountered stated in item III also contributed to project delays.

III. PROBLEMS ENCOUNTERED ACTIONS TAKEN


Water seepage along the perimeter of After locating all the sources of water seepage that

IV. REQUISITION OF CONSTRUCTION FUNDS


May we request the amount of Two Million Thirty-One Thousand Four Hundred Forty-Two &
29/100 as scheduled by contract to supplement up to 46% of work progress

V. LIST OF WORK VARIATIONS/ CHANGE ORDER

The contractor is constantly in coordination with the designer, Ar. Alvin Matias, it is inevitable
for changes and revisions as the construction work progresses. As stated in the contract, we will
present all work variations, additions, and revisions with corresponding deductive and additive
amounts, which will serve as a reference to the overall project completion. Kindly refer to the
attachments please for the said work variation and change orders.
