INFORMED CONSENT FORM

Please read this consent agreement carefully

Literacy Training Service is geared towards activities that have social impact through
activities that could contribute to health, education, environment, entrepreneurship, safety,
recreation and morals of the citizenry. NSTP-LTS2 stressed the importance of youth involvement
in broad programs or activities that will benefit the people through the basics of reading, writing,
and numbers.

Objectives:

1. To develop a sense of volunteerism in serving others by reaching out and engaging in a

free service to the people in our community.
2. To share and extend the skills and knowledge in the community especially in education
sector.
3. To prepare and train students to work effectively in the new normal set-up.

What will the beneficiaries do in the study: The learners from the partner school will be the
primary recipient the tutorial program. They will have free learning materials and snacks during
the session.

Payment: The NSTP-LTS students will not receive any payment for this Tutorial Program since
this will be part of their voluntary partnership and requirement in the course.

hereby give my consent for my child to participate in this Literacy Training Service Tutorial
Program.

I am aware that I should keep a copy of the Informed Consent Form for future

reference. I understand that:

▪ My child’s participation is part of the requirement in completing the NSTP

LTS2 course.
▪ There will be a minimum monetary contribution for the learning materials
and snacks of the learners who will attend the program.
▪ The tutorial program will be done face to face at every Sunday (March-May)
▪ The individual information of my child will be kept confidential.
