The intent of this notice is to inform you that Supervisor have not been satisfied with you for the
reason that are indicated below and formally give you a notice of termination .

BACKGROUND:
You have warning with your duties , On 16th of may 2016
and the final warning on 9 of dec you received the same complaint from the supervisor

OFFENSE:
1. Refusing to follow the instructions of your immediate supervisor.
2. Unsatisfactory work performance.
3. Always late for work
RESULT OF OFFENSE/ PENALTY:
After careful consideration of the matter and looking at your history of misconducts management has
decided to terminate you with immediate effect . This action has been taken as per federal law of the
UAE , Article 120 , Clause – E.

copy of disciplinary action.
