AFFIDAVIT

I, ANGELICA E. GANDIA, of legal age, Filipino, single, and a resident of 

Dasmariñas, Cavite, after being sworn to in accordance with law, hereby depose and state
that my old residential address, to wit:

1. That I am the authorized representative of MC DONALD MESINA owner of the

business named BUBBLE WASH SELF SERVICE LAUNDRY under business clas:
Contractor – Laundry Services located at #52 Governor’s Drive Langkaan,
Dasmariñas, Cavite;

2. That I hereby attest to the fact that BUBBLE WASH SELF SERVICE LAUNDRY was
started from year 2019 up to this date; and

3. That the reason why the business is not registered on year 2019 because the only
operator of the business is the owner himself and there are no employees in that
year;

4. I am executing this Affidavit to attest to the truth of the foregoing and for whatever
legal purpose this may serve.
