Instructions

General
1. This form is to be completed by inspectors conducting inspections on Exera Guard Posts in
Myanmar. This form is divided into Five Parts: Instructions, Identifying Information, Core
Competencies, General Competencies and Health, Safety and Environment (HSE).

2. Completely fill out all fields in the form. If a space is left blank or a question unanswered, write
N/A and provide an explanation why the criteria is inapplicable in the “Remarks” section at the end
of the form.

3. Complete the entire inspection, even if a post has failed a Core Competency.

4. The results of the inspection are confidential and may only be shared outside Exera with
permission from the General Manager.

5. You may present the results of the inspection to the post only upon completion of the entire
inspection.

Scoring

6. An answer of “no” for any Core Competency criteria results in an automatic fail and must be
followed by a Critical Non-Conforming Service Report.

7. A grade of less than 60% across all General Competencies results in a fail and must be followed
by a Non-Critical Non-Conforming Service Report.

8. The HSE inspection is not scored for service quality and does not contribute to the “pass” or “fail”
of the inspection. Regardless, it must be completed in full.

9. Scoring for General Competency is achieved by dividing the number of “yes” answers by the
total applicable questions.

10. Submit the hard copy of the completed inspection form by mail or in person to the following

of the inspection. Regardless, it must be completed.
1. Is shelter of an appropriate kind provided for guards? YES

Things need to be done in site

Some time there are meeting at the office . At that time visitor pass is not enough to be issued to the guests .
10 visitor passes need to be issued to the Guard post.
