What to Check and What to Report?

As far as practicable, attending surveyor to confirm that sheaves are in apparent


satisfactory condition and free from any damages. Special attention must be paid to the
sheave bearings and there should not be any signs of the same sliding out. As far as
practicable, attending surveyor to verify the sheave clearances. To confirm the sheave
below guidance may be adopted to confirm if the sheaves are in good condition.

Attending surveyor may adopt his own method and to verify the sheave clearances if
any. If, however condition of the sheaves or the clearances are not in accordance with
the makers recommendation, this check is to be marked as “NO”.

Are crane/derrick safety devices operational and regularly tested?

What to Check and What to Report?

Attending surveyor to confirm that cranes are regularly tested and in accordance with the
manufacturer recommendation. As far as practicable, attending surveyor to practically
witness the testing of the safety devices and confirm that same are operational. If
witnessing the tests is not possible, attending surveyor to confirm the documentation and
the PMS activities and ascertain that safety devices are checked on a regular basis.
As a minimum usually all cranes are provided with the following type of safety devices
which may be tested (if possible) at the discretion of the attending surveyor.

 Crane level indicator (If provided)


 Boom stops (except for derricks and hydraulic booms)
 Jib stops (if a jib is attached), except for derricks
 Locks on foot pedal brakes
 Integral holding device/check valve on hydraulic outrigger jacks and hydrau-
lic stabilizer jacks
 Rail clamps and rail stops for equipment on rails (If provided)
 Horn (either built into the equipment or on the equipment and immediately avail-
able to the operator)
 Boom hoist limiting device (If Provided)
 Luffing jib limiting device.
 Automatic or warning-type anti two-blocking device (If Provided) (Hook
Hoisting limit switch)
 Boom angle or radius indicator (If provided)
 Jib angle indicator if the equipment has a luffing jib
 Boom length indicator if the equipment has a telescopic boom (If Pro-
vided)
 Load weighing and similar devices (If Provided)
 Automatic overload prevention device, load weighing device, load mo-
ment (or rated capacity) indicator, or load moment (rated capacity) limiter (If
Provided)
 Hoist drum rotation indicator if the equipment has a hoist drum not vis-
ible from the operator’s station (If Provided)

Is the slew bearing wear being regularly monitored e.g. by
grease sampling or rocking test?

What to Check and What to Report?

Attending surveyor must verify that rocking test or grease sample test is conducted on
and all clearances obtained are in line with the manufacturer recommendation. Rocking
test of deck crane is usually carried out to ascertain the wear down of the sleeve bearing
of the deck cranes by measuring the play (relative movement) between the inner and
outer bearing race. This measurement indicates the rate of wear. If practicable attending
surveyor may witness the rocking test of any of the cranes at his/her discretion.

significant corrosion?

What to Check and What to Report?

As far as practicable attending surveyor should closely monitor the holding down bolts
and the slewing ring. It should be ascertained that bolts and the slewing ring is free
from signs of excessive corrosion. If there are potential signs of poor condition or lack
of maintenance in these areas, this check is to be marked as “NO”.

Is the hook safety latch spring operational and the hook movement
free and the hooks-are free from deformation, excessive wear or
fractures?

What to Check and What to Report?

As far as practicable attending surveyor to check the condition of the hooks and confirm
that same is free from any visible damages and has free movement. Safety latch on the
hook should also be checked and should be in good condition. If these checks or any
other check at the discretion of the attending surveyor is not meeting criteria, this check
is to be marked as “NO”.

Are the crane / derrick electrical / hydraulic system free from
apparent defects or leakages?

What to Check and What to Report?

Attending surveyor to ascertain that all electrical and hydraulic components are in
good condition and fully intact. It is also to be confirmed that hydraulic pumps,
motors and electric fans are free of excessive corrosion and in apparent working
condition. Other appliances, such as oil storage tank and electric motor should also be
checked and confirmed that they are in sound condition. If any of these checks or any
other checks as deemed necessary by the attending surveyor are not meeting criteria,
these checks should be marked as “NO”.

Are crane access ladders and platforms in apparent satisfactory
condition and allow safe access?

What to Check and What to Report?

Attending surveyor to confirm that all access platforms and ladders attached to the
cranes structure and other areas are in apparent satisfactory condition.
As a minimum attending surveyor may check on the following:
• Ladders should not have any step wasted to an extent where it may break due
to weight
• Ladders should be free from excessive wear and wastage
• Ladders should not have any missing steps
• Ladders should not have any wasted welds/joints specially in way of steps
• Ladders should be free from grease or any other material which may result in
poor grip
• Ladders should be straight and free from deformations
• Where vertical ladders to lower decks are not in a direct line, a safe
intermediate landing should be provided.

If the above checks or any other check as deemed necessary by the attending
surveyor, this check is to be marked as “NO”.

Is the Jib mounting (hinges, luffing cylinder connection)
adequately greased and free from wastage?

What to Check and What to Report?

Attending surveyor to closely check the condition of the jib. Jibs should be free from
any sign of excessive corrosion. If luffing cylinders are connected to the jib, hinges
should be free of corrosion or any signs of thin down of material. There should not be
any signs of oil leakages from the luffing cylinder and RAM seals should be in apparent
satisfactory condition. Hydraulic lines to the RAM should be free from excessive
wastage and should not show signs of leakage. There should be apparent clear signs
that jib is well maintained, and grease lines are maintained in good order.

If above checks or any other check as deemed necessary by the attending surveyor is
not meeting the criteria, this check is to be marked as “NO”.

Is the operator cabin clean and free of unwanted objects?

What to Check and What to Report?

Attending surveyor may check the following items when inspecting the operator cabin
and ascertain that below required items are available in the operator cabin.

• There should be a Safe access and egress are provided to the crane operator
• A load capacity chart should be displayed in the operator cabin.
• Operator crane cabin should be provided with a locking mechanism to prevent
unauthorized entry.
• A safety bar should be fitted across the operator’s cabin window where there is
likelihood of the operator falling through it.
• Fire extinguisher should be fitted for use and provided in the operator cabin and
should be properly secured
• An updated Operation instruction should be available in the cabin.
If above items or any other as deemed necessary by the attending surveyor is not
meeting the criteria, this item is to be marked as “NO”.

and operational in the operator’s cabin?

What to Check and What to Report?

Attending surveyor to confirm that means to escape from the operator cabin is clearly
marked and full accessible. Means should be free from any obstruction and clearly
marked. Instructions on how to access the apparatus must be clearly posted at a visible
location and in close proximity to the emergency escape.

If above items or any other as deemed necessary by the attending surveyor is not
meeting the criteria, this item is to be marked as “NO”.

sound and working condition?

What to Check and What to Report?

Attending surveyor to confirm that work console and all associated components such
as Joysticks, various meters, alarm lights, digital/analogue displays, etc are in
apparent satisfactory condition and do not show any signs of malfunction of damage.
If the attending surveyor is not satisfied with the condition of these equipment, this
check is to be marked as “NO”.

What to Check and What to Report?

Attending surveyor to confirm that operator cabin windows are clean and clear. There
should not be any signs of damage to the glass nor to the window frame. Special
attention must be paid to the window frame gaskets, and there should not be any
apparent signs indicating the gasket rupture or damage. Material in way of the window
frames should also be checked for thing down or any cracks. There should not be any
cracks on the window.

If any of the above checks or any other check as deemed necessary by the attending
surveyor is not meeting criteria, this item should be marked as “NO”.

hanging in the cabin, this check is to be marked as “NO” by the attending surveyor.

What to Check and What to Report?

Cabin structure should be closely inspected and checked for any apparent signs of thin
down of material, holes, visible damages, buckling, heavy corrosion, wastage of
material etc. Areas in way of the window frames should be closely inspected and in line
with 1.12.3.15. If these checks or any other check as deemed necessary by the at-
tending surveyor is not meeting the criteria, this item is to be marked as “NO”.

Is the vessel maintaining a critical spare parts list for crane parts?

What to Check and What to Report?

Attending surveyor to confirm that vessel is maintaining a system to identify and


maintain a minimum stock of certain spares parts which are recognized as an
essential item which must always be kept on board as a minimum to deal with an
unforeseen breakdown.
These spare parts should be readily available and as far as practicable, vessel should
have a system to recognize such critical items readily.

If the vessel is not maintaining such spares or is low in stock this check is to be
marked as “NO” by the attending surveyor.

Is the cargo gear certificate issued by class valid?

What to Check and What to Report?

For vessels assigned with cargo gear notations, they should be maintaining cargo gear
record book on board duly endorsed by Class. Further vessel should always maintain a
valid annual cargo gear certificate and a 5 yearly cargo gear load test certificate. If
vessel is not in possession of valid class cargo gear certificate, this check is to be
marked as “NO”.

What to Check and What to Report?

Attending surveyor should sight crane wire certificates on board and confirm that an
individual wire certificate is available for each wire, for e.g is 5 crane wires are present
on board then there should be 5 separate crane wire certificates. Attending surveyor
should also confirm that spare crane wire on board have a separate certificate.
If the vessel is not in possession of a certificate or is missing out on a certificate for
any of the wires, this check is to be marked as “NO” by the attending surveyor.

Are there any planned maintenance activities that are falling
overdue in accordance with vessel PMS?

What to Check and What to Report?

Attending surveyor to verify that there are none PMS activities falling due for cranes.
All activities as listed out in the PMS activities should be completed within the due
date and if they could not be completed, necessary approval from the shore
department should be taken. In case maintenance is found to be overdue and not
approved by Shore department with relevant documentation then this check is to be
marked as “Yes”.

Is the vessel PMS crane activities relating to makers instructions?

What to Check and What to Report?

Attending surveyor should confirm that PMS activities should be in accordance with
the makers instructions as a minimum. Additional activities in the PMS basis owners
experience is highly recommended, however all makers instruction/activities should
be included in the PMS system. Attending surveyor may verify the activities at
random in accordance with the makers manual and confirm that they are matching. If
makers recommended activities are not included in the vessel PMS, this check is to be
marked as “NO”.

instructions?

What to Check and What to Report?

Attending surveyor to confirm that crane LO samples are landed for shore analysis in
accordance with makers instructions. Further attending surveyor confirm with the
vessel if there exists a system to conduct LO analysis using the onboard test kits. If
there exists no procedure for testing LO ashore or if the samples have not been
landed within the due date, this check is to be marked as “NO” by the attending
surveyor.

What to Check and What to Report?

Attending surveyor to confirm that all values indicated towards individual oil samples
landed, are within the specified range towards a specific standard. If the values are
out of range, attending surveyor to confirm the actions taken to mitigate the out of
spec values. If there is no evidence of corrective actions taken, then this is to be
marked as “NO” by the attending surveyor.

What to Check and What to Report?

Close attention should be paid to hydraulic hoses and connectors. There should not
be any signs of heavy wastage or signs of leakages in way of connectors for hydraulic
hoses. Condition of hoses should be satisfactory and free from signs of damage or
signs of wear. There should not be any indication of leakages from the hoses. If
hoses or connectors are not meeting the criteria, this check is to be marked as “NO”.

Hydraulic blocks should be checked for signs of excessive wastage or corrosion. There
