AUTHORIZATION LETTER

To Whom It May Concern,

I, Richelle C. Cumilap, hereby authorize Joel N. Empil, who is my husband, to claim my

savings money on my behalf. Joel N. Empil is allowed to claim the total amount of Php
19,170.00 from ASA located at Tudela, Misamis Occidental. I am personally unable to claim
the money myself because of my current job here in Manila under contract and unable to
travel there immediately.

To support my claim, Joel N. Empil shall be presenting his identification card for your
verification. I have also included my identity card in this letter to serve as proof that my
representative has the authority to claim the savings money on my behalf.
