1.0 SCOPE AND PURPOSE

This guide and template is to be used in conjunction with the requirements of Site Survey,
Installation and Commissioning.

The worksite survey is a valuable document. It projects the Company to the customer and is
a reflection of our professionalism.

Consistency of quality is important and this document has been made to guide and assist the
survey engineer in the methodical collection and presentation of the data.

By taking time to complete all sections, much of the final report will be completed. This
workbook will then be retained as a permanent record of the survey procedure.

3.0 SAFETY STATEMENT

Safety is paramount. All applicable legal and Company Safety Standards must be
adhered to at all times. Where any procedure gives cause for uncertainty or concern,
clarification must be sought from responsible Company representatives before
proceeding.

Refer to SCOMI Health, Safety and Environmental Manual and its supporting
procedures.

Scomi Oiltools personnel must comply with the client or site/location operators HSE
requirements and practices when conducting a survey.

PPE POLICY

Due to the nature of the process carried out aboard an offshore facility, personal protective
equipment (PPE), is an extremely important feature of working in the area.

Standard Scomi Oiltools Issue: Hardhat, ear protection, coveralls, safety boots, glasses
and gloves must be worn at all time onboard the facility.

Be aware of site specific hazards, such as fumes or dust, and wear the appropriate special
PPE.

SAFETY INDUCTION

All Scomi personnel that are to carry out the Survey Procedure at the onshore/offshore facility
must firstly have attended the relevant location safety induction courses.

PERMITS REQUIRED

Clarification must be sought from responsible company representative e.g. OIM regarding any
work permits you may require (Hot work for taking photographs).

SITE SPECIFIC HAZARD

Clarification must be sought from responsible company representative e.g. OIM regarding
specific hazards before commending with the survey.

6.0 RIG FAMILIARISATION

It is recommended that prior to the survey being carried out, the engineer familiarises himself
with the workplace.

- Prior to conducting survey: Walk the system.

- Where possible, get escorted tour by Toolpusher.

- Examine proposed worksite

Discuss job with Chief Engineer and point out where you intend to burn or weld. Ensure
you know what is on the other side of any bulkhead, roof or floor you intend to weld or
burn. Look for fuel tanks, electric cables, and possible gas filled voids. Ensure the steel
work that is to support all new equipment is suitable. Consider alternative worksite.
REQUEST RIG SPECIFIC DRAWINGS, GA- of WORK SITE, AUTO-CAD if POSSIBLE

- Examine adjacent facilities (Water, Base Oil, Air Line, Electrical Supply, etc).

- Establish Certifying Authority (if possible) – (OIM can assist) ABS, DNV, NORSOK etc.

Discuss the job in hand with Chief Electrician and obtain rig power supply details. Volts,
frequency? Inform the electrician of your power requirements and ascertain what will be
necessary to provide power for the new equipment. Clarify zone areas, should replacement
switch gear be in a safe area (SCR?)

7.5 Photographic requirement

7.6 Rig Drawings

Obtain GA drawing, Piping and Instrument Diagram (P&ID) of system whenever
possible.

To best understand the nature – Scope of the Survey, sketch the worksite using the
following templates.

Drawing should include but not be restricted to:

- Space available with lines

- Equipment inserted

- Access required, Installation path, Hoist points

- Obstructions, height, width, pipework, service location, water, air, electrical

- Working clearance, (Operating/Maintenance).

- Support structure

- Modifications to rig facilities required.

- All dimensions must be clear, Centre line of pipes/beams/structure? Alternately

overall dim’s. RECORD ALL RELEVANT DIMENSIONS, (There are never too
many dim’s).

Notes on drawing:-

Attempt to address safety as a priority. Where possible, carry out a Risk Assessment
to identify potential Hazards of the installation or subsequent operation/maintenance of
the equipment.

Drawings can be duplicated with different information shown. Do not clutter a single
drawing.

Make field sketches of the installation. Take lengths, widths and heights. Draw a plan,
end and side view of proposed installation. Draw single lines for interconnecting
pipework with arrows to show flow direction. Identify suction and discharges, pump
location, etc. A proper drawing can be produced in town. Also detail any
modifications to existing pipework or steelwork.

Additional requirements for equipment designed for lifting persons.

Has equipment been thoroughly examined within the last 6

months?
Is a device fitted to prevent personnel falling from carrier?
Is equipment clearly marked as being suitable for lifting
persons?
Either:- Is a device fitted to prevent carrier falling, in case
main support fails?
Or:- Does carrier have a suspension rope, wire or chain at
least twice as strong as the marked SWL, which is
inspected every day?

Is there documentary proof of regular maintenance?
Can all required maintenance be carried out?

standby/watchkeeping personnel, adequately trained?
Has equipment been inspected before first use?
Has sufficient, easy to understand information been
provided to enable safe use of the equipment?
Have risks of using this equipment been fully identified,
assessed, documented and controlled?
Is protection provided to protect persons from danger
caused by moving, rotating, hot or cold parts?
Are controls provided to start, stop and isolate the
equipment from power sources, in both normal and
emergency situations?
If first supplied after 1/1/95, does item have a CE
mark?
Are systems in place to ensure no persons are
exposed to risk as a result of equipment starting or
operating?
Where appropriate, have visible or audible warning
devices and barriers been put in place?
Is there sufficient lighting?

Additional requirements for self-propelled, mobile equipment.

Are passengers prohibited from riding on equipment?

Is equipment provided with a means to stop un-
authorised use?
Are controls fitted for braking and stopping?
Are there emergency controls to brake and stop if main
controls fail?
Does the equipment operator have an unrestricted view
of the work environment?
