ANDONGA, VOLUNTARILY GIVEN TO

POLICE STAFF SERGEANT JON MISHA SUAYBAGUIO, AT THE OFFICE OF
INVESTIGATION AND DETECTIVE MANAGEMENT SECTION OF SAN FRANCISCO
MUNICIPAL POLICE STATION, SAN FRANCISCO AGUSAN DEL SUR IN
SUPERVISION OF POLICE MAJOR LYKA M LUAREZ, CHIEF OF POLICE.

Affiant was apprised of his rights that he could not be compelled to answer question/s
which may tend to incriminate or degrade himself, after which he testifies as follows:

Q1 : After being informed of your rights do you still wish to proceed with this investigation?
Ans : Yes, I do.
Q2 : Do you swear to tell the truth, the whole truth and nothing but the truth only?
Ans. : Yes, I do.
Q3 : If so, please state your name, age, address, and other personal circumstances.
Ans. : I, Balong C. Andonga, 26 years old, single, a resident of P-2, Alegria, San Francisco,
Agusan del Sur. I am a friend of the victim, Jeckson L. Baja.
Q4 : Why are you executing this affidavit?
Ans. : To testify for Murder against Mark R. Elangga.

Q5 :When and where the incident happened?

Ans. : On April 25, 2023 at exactly 5:30 pm in front of HIGHWAY BAR located at P-2 Brgy 3,
San Francisco, Agusan del Sur.
Q8 : Can you please narrate what happened?
Ans. : On April 25, 2023 at 1:45 PM, I went to the Highway Bar because I was invited by the
victim to celebrate his birthday together with our friends. At around 3:00 o’clock in the
afternoon, while I was busy talking with my other friend, Mark R. Elangga and Jeckson
M. Baja were having a conflict and it resulted to a fist fight. After that incident, the
suspect went out on the Highway Bar and a few minutes later, our friends went home.
Q9 :What’s next?
Ans. : I accompanied the victim and helped him to calm down. We stayed in the bar for about
two hours but then he insisted to go home so I let him. Jeckson then went out to the bar,
but he did not notice that the suspect approached him from behind and stabbed the victim
five (5) times in the right portion of his stomach. The victim fell on the ground.

Q10 : What’s next?

Ans : After the commission of the crime, the suspect immediately left the incident area.
Q11. : The investigator has nothing more to ask you, do you have anything to add or retract
from your statement?
Ans. : Nothing at the moment.
Q12. : Do you confirm and affirm that your foregoing statement is based on your own and
personal knowledge and not or fear, intimidation, coercion, threat, force, couched and/or
a promised of reward what so ever?
Ans. : Yes, I do.
Q13 : Will you sign your statement freely and voluntarily?
