POLICE OFFICE STATION I, NAGA CITY, IN 15 TH DAY OF MARCH 2023,
AT AROUND 2:00 IN THE AFTERNOON.

PRELIMINARY: Do you swear to tell the truth, the whole truth, and nothing but
the truth in this Investigation?
ANSWER: Yes, sir.

Q 1: Please state your name, age, and other personal circumstances?

A 1: AL-AMED y KHAN, 28 years old, single, Self-employed, and a residence of
Zone 3, San Jose, Pili, Camarines Sur.

Q 2: Why are you here in the office of the Investigation Section and being
Investigated?
A 2: To file a complaint against JUSTIN PONTANAL of Zone 3 San Jose, Pili,
Camarines Sur

Q 3: What is your complaint against JUSTIN PONTANAL?

A 3: He robbed our house sir.

Q 4: Will you narrate the facts surrounding that incident?

A 4: On or about 2:00 am at my resident when I was asleep, I heard a loud
banging in the other room of my house, when I checked it I saw unidentified
man wearing a black mask forcefully ramming our wooden window to escape the
room.

Q 5: What happened next?

A 5: Then I caught him with a large bag containing I think the stolen item which
are 1pc Cellphone worth 15,000.00php and laptop worth 35,000.00php.

A 6: When I tried to alert the neighbors thru shouting, the suspect points me the
gun that he carried.

Q 7: What happened next?

A 7: To protect myself, and because I was threatened, I didn’t shout because I
was also intimidated by the gun.

A 8: After the suspect escaped with the motorcycle, we found the fallen driver’s
license where he parked his motorcycle.
