My Warmest Greetings!

My husband and I are encountering a greatest challenge in our marriage life. We have been
married for almost four years now and we have been working it out to have a baby. However, my
husband was diagnosed to have Varicocele (the diagnosis is attached to this letter) which hinders
our desire to have a baby. He was advised to undergo for an operation however we were afraid
that there may be other consequences since the doctor said that there is no assurance that this will
be totally treated. That is why we first undergo for therapy for almost 8months now to gradually
eliminate the blocking of the veins. This is also the reason why I frequently leave from my work
to support my husband. Unfortunately, last January 8, 2020, we had sperm count analysis to find
out if there are other improvements from our therapy. And sadly, we found out that he has zero
sperm count result (attached is also the result). My husband is somehow suffering from
depression as what I have observed from his attitude lately. He is very short tempered,
indisposed and apathetic. I wanted to be with him most of the time to let him feel that he is
always wanted and loved. We will also be looking for another doctor to seek for another opinion
and we will be doing it as soon as possible. We have plan to undergo Intrauterine Insemination
(IUI) in the future which is a fertility treatment that involves placing sperm inside a woman's
uterus to facilitate fertilization to conceive a baby as per advised by my OB since I am polycystic
but we need to address first his diagnosis. I really wanted to do anything to help my husband and
fulfill the desire of our hearts because I am already aging.

For these reasons, I am humbly asking for your approval to allow me to leave from my work
from January 15, 2020 until this school year ends enable for me to concentrate helping my
husband. He needs my support as I am the only person who could understand his situation since I
am his wife. I need to prepare him physically, mentally, spiritually and emotionally. I do not want
also to neglect my obligation as teacher that is why I am formally asking for your approval. As
much as I wanted to report to school, but I cannot leave my responsibility to my husband
especially that we have this challenge in our life.

I will be forever thankful that I am part of Banisil National High School. This school is already a
perfect working station for me because it is a performing school which always contributes honor
and good reputation to the Department of Education. Additionally, the teachers are very easy to
work with and this working station gives me a better environment to improve my profession.
However, I am asking for your recommendation to be transferred to any schools near to our
residence for the following reasons:

1. I am a resident of Brgy. San Isidro, General Santos City and I live there together with
my husband so it is be more convenient if I will be working within our locality.
2. I will be more productive and effective teacher if my working station is near to my
residence.
3. Considering the distance from our residence, it is risky in my part to travel everyday
and also it is inevitable to come late to school.
4. I have been married for almost three years and I have sensitivity in conceiving a
child. We have undergone several workups and take medications however my daily
travel from our residence to my current station and vice versa contributes stress to my
uterus which gives me misfortune to get pregnant. Our treatment and medication are
still on going and we are seeking for your consideration and understanding about our
work-up.

For these reasons, I am humbly appealing to your office to consider my intention to transfer.
