The attached Purchase Order and Notice of Award having been approved, notice is
hereby given to C.T. SILVA BAKERY that the delivery may commence/ proceed the SUPPLY
AND DELIVERY OF BREAD FOR SBFP 2021 within SEVEN (7) CALENDAR DAYS.

Upon receipt of this notice, you are responsible for delivering the goods awarded to
your company as a result of Canvass where you submitted the lowest calculated and responsive
bid. The said goods/services shall under the terms and conditions stipulated in the specifications
of the project, and must be delivered within SEVEN (7) CALENDAR DAYS from the date this
notice. After the deadline of delivery, the SAN RAFAEL ELEMENTARY SCHOOL will
deduct one-tenth (1/10) 0f 1% of the Contract Price from its Obligated Amount in each day
failed of delivery. Actual receipt of the goods delivered shall be performed by the Supply Office
of the Division.

Please acknowledge receipt and acceptance of this notice by signing both copies in the
space provided below. Keep one copy and return the other to the SAN RAFAEL
ELEMENTARY SCHOOL.
