This is to CERTIFY that quoted hereunder is the Blotter /Entry specifically reflected in the
Police Blotter of this Station entered on December 27, 2022 the following Entry exists to wit;

Report of Lost Wallet

(Non-crime)

That on or about 9:30 AM of December 27, 2022, one(1) FELIPE JAVIER BARBA of 59
years old, married (DOB- May 1, 1963) Filipino and resident of Brgy Numero, Mabitac, Laguna,
personally appeared before this office and reported that his wallet was lost on December 25,
2022 within the vicinity of Mabitac AOR. According to the reportee he noticed that his wallet
was loss when he arrived home from Brgy San Miguel, Mabitac, Laguna. Said wallet containing
his professional driver’s licence, GSIS ID, UMID ID, Pantawaid Pasada ATM card and cedula.
Despite of diligent search and effort to locate said wallet, still could no longer find and believe
that is now lost beyond recovery.

Issued this 27th day of December 2022 in Mabitac, Laguna, Philippines for whatever
legal purposes it may serve.
