FORMAL OFFER OF DOCUMENTARY EXHIBITS

COMES NOW, the petitioner, through the undersigned counsel and

unto this Honorable Court, most respectfully submits this Formal Offer of
Documentary Exhibits with their corresponding purpose, to wit:

1. Exhibit “A” – SPECIAL POWER OF ATTORNEY;

This is offered to prove that Virgilio C. Acuna and Gloria R. Acuna authorized
Amelia Centillo Retuerto to appear, testify, sign and represent them before any
court or tribunal of competent jurisdiction for filing of the petition for the issuance
of TCT No. T-63235 in lieu of the one which was lost and to execute an affidavit
of loss.

2. Exhibit “B” – AFFIDAVIT OF LOSS;

This is offered to prove that original copy of TCT No. T-63235 was lost in the
possession of Amelia Centillo Retuerto and despite diligent efforts to locate it, the
same cannot be found anymore.

3. Exhibit “C” – CERTIFIED COPY OF TCT NO. T – 63235 WITH THE

CORRESPONDING ANNOTATION OF THE AFFIDAVIT OF LOSS;

This is offered to prove that the Affidavit of Loss executed by the petitioner was
annotated at the back of the title TCT No. T-63235 as per entry no. 2022001542.

4. Exhibit “D” – CERTIFICATION FROM THE REGISTER OF DEEDS

THAT TRANSER CERTIFICATE OF TITLE NO. T- 63235 IS STILL
EXISTING AND INTACT IN THEIR OFFICE;

This is offered to prove that as per the records of the Register of Deeds (ROD),
Amas, Kidapawan City, they have the original copy of the above title.

5. Exhibit “E” – JUDICIAL AFFIDAVIT OF AMELIA CENTILLO

RETUERTO;

This is offered to serve as the direct testimony of the witness.

 6. Exhibit “F” – CERTIFICATE OF POSTING;

This is offered to prove that the jurisdictional requirements of posting have been
complied with.

PRAYER

WHEREFORE, premises considered, it is most respectfully prayed of this

Honorable Court that after due notice and hearing, an order be issued directing the
Register of Deeds of Amas, Kidapawan City to re-issue Transfer Certificate of
Title No. T- 63235 in lieu of one which was lost.
Other reliefs just and equitable under the circumstances are likewise prayed
for.
