I. INTRODUCTION:

In order to provide a trustworthy assurance mechanism for learner information,
guarantee the quality and timeliness of school reports, and reduce the resources used for
clerical and records management, the Department of Education (DepEd) hereby
prescribes the standard process and protocols in the preparation, evaluation, and updating
of school forms conducted at the end of every school year.
II. OBJECTIVES:
A. Terminal Objective:
By the end of the session, participants will be able to show skills in the
positive use of ICT to facilitate learning and teaching.
B. Enabling Objectives:
Identify the different school forms and their uses;
Acquire knowledge on the use of the Learner Information System (LIS);
Demonstrate knowledge of providing timely, accurate, and constructive
feedback using the different school forms and the LIS.

the K–12 Program, were also taken into account. The use of the modified forms will
enable all public school teachers and administrators to concentrate on their main
responsibilities, which is delivering curriculum.
