NARRATIVE REPORT on
Unstoppable Growth through Meaningful Learning in Different Areas of Development
(UGMAD): A School – Based Learning Action Cell (SLAC) of Malubog Integrated School
Teaching & Non – Teaching Personnel for S.Y. 2023
SESSION 5: School Forms and Learner Information System (LIS)

I. INTRODUCTION:

In order to provide a trustworthy assurance mechanism for learner information,
guarantee the quality and timeliness of school reports, and reduce the resources used for
clerical and records management, the Department of Education (DepEd) hereby
prescribes the standard process and protocols in the preparation, evaluation, and updating
of school forms conducted at the end of every school year.

II. OBJECTIVES:
A. Terminal Objective:
 By the end of the session, participants will be able to show skills in the
positive use of ICT to facilitate learning and teaching.

B. Enabling Objectives:
 Identify the different school forms and their uses;
 Acquire knowledge on the use of the Learner Information System (LIS);
 Demonstrate knowledge of providing timely, accurate, and constructive
feedback using the different school forms and the LIS.

III. ACTIVITIES UNDERTAKEN:


The Malubog Integrated School conducted its fifth School – Based Learning
Action Cell on the 9th day of June, 2023. The session was facilitated by the school’s
master teacher, Mrs. Pamela E. Cabusao.
Following her presentation of the session's goals, Mrs. Cabusao went on to
thoroughly discuss each Department of Education school form in turn. Since we use
these forms every academic year, Mrs. Cabusao made sure we were familiar with
each one by heart. In accordance with the government's initiative to provide
affordable, relevant, and high-quality education to all Filipino children and youth, the
department adopted modified school forms (SFs) and started using them at the
conclusion of the 2013–2014 school year in all public elementary and secondary
schools. Mrs. Cabusao emphasized that end users at all agency levels participated in a
thorough review, validation, and consultation process for the amended SFs.
Consistency and compatibility with the department's goals and activities, particularly

the K–12 Program, were also taken into account. The use of the modified forms will
enable all public school teachers and administrators to concentrate on their main
responsibilities, which is delivering curriculum.

IV. IMPACT OF THE ACTIVITIES/RESULTS:


One of the crucial tasks completed at the conclusion of every School Year (SY) is
the production and inspection of school forms, which is done to guarantee the
accuracy and consistency of learner information. This session was timely since it is
nearing the end of school year and school reading of forms will be conducted by the
end of the month. This session has supported my growth as an agentic/ empowered
teacher by educating me to be cautious and precise when carrying out the different
school forms introduced me to some school forms which I have not known ever since
I worked in this institution. Most importantly, I was reminded that I am in charge of
the students' sensitive information and I am compelled to keep all learner documents
confidential.
