This Monitoring Tool shall be used to assess the readiness of the school for the
Regional Standardized Assessment 2023. The data gathered will be used as baseline
data in the planning and provision of technical assistance to the school.

2. The school secured a copy of the Implementation Plan
for the Regional Standardized Assessment.
3. The Division Testing Coordinator, ITO, and School Head
conducted an orientation for learners and parents.
4. The School Head assigned Test Proctors from different
grade level/s.
5. The Division office conducted an orientation for the
School Heads and proctors.
6. The School has comprehensively profiled the examinees.
7. The list of examinees was provided by the school.
8. The School initiated an activity to ensure the attendance
of the examinees on the assessment day.
9. The Division Testing Coordinator, Division Education
Program Supervisors have inspected the school.
10. The school designated two holding areas, one for each
test session. These rooms shall be utilized as snack and
lunch areas for the learners.
11. Extra chairs and tables and unnecessary furniture
were removed from the testing room.
12. The Testing room is well-ventilated and has enough
chairs that are 1 to 2 meters apart.
13. The required number of test takers for the school is
achieved.
Small (Schools with less than or equal to 440 learners) = 15 samples
Medium (441-840 learners) = 20 samples
Large (841-1240) = 30 Samples
Very large (1241-Above) = 35 Samples

SPTVE, SPFL).
15. The are only at most 20 test-takers in the classroom.
16. The proctor facilitated the checking of the test and
taking the frequency of correct answers per item.
17. The proctor accomplished the template for the
“Proficiency Level and Least Mastered Competencies
Report”
18. The proctor was able to encode the results in the
region-made Google form for RSA Results.
