EXAM INSTRUCTIONS. PLEASE READ AND UNDERSTAND THE INSTRUCTIONS BELOW. Not reading the
guidelines/instructions may compromise your examination

1. Be in your ASSIGNED ROOM otherwise you will be marked absent even if you took the exam
in another exam room.

2. Be in PROPER UNIFORM for the exam, complete with nameplate and ID.
 3. BRING YOUR OWN PEN for signature in the attendance sheet. Minus points will be given for
borrowing pens.

4. ONLY 1 device, either Laptop, iPad, or tablet is ALLOWED in taking the exam.

5. Bring your OWN wifi/mobile data/internet connection.

6. Bring your own charger/battery pack/extension cord.

7. Ensure that your device is FULLY CHARGED before the start of the exam. To safe battery life,
turn on your device only when the exam is about to start.

8. Use of mobile phones (other than as hotspot) during the exam is PROHIBITED.

9. NO other browser should be open on your laptop/ipad/tablet EXCEPT MS FORMS.

10. NO chatbox or app should be open in your device.

11. Make sure you are able to sign the attendance sheet prior to dismissal. If you fail to sign the
attendance sheet, you will be marked ABSENT and your exam will be VOID.

The following acts/actions constitute CHEATING equivalent to a grade of ZERO:

 1. Use of another person’s account credentials (username and/or email and password) to login.

2. Allowing someone else to use your MS account login credentials to take the exam.

3. Opening/Viewing of a browser, tab, or window OTHER THAN the exam

4. Opening/Viewing ANY document (pdf, word, pages, and the like) or app (viber, whatsapp,
telegram, messenger, wechat, and the like) during the exam

5. Use of additional devices to take the exam (other than what was specified)

6. Talking/whispering to anyone (even mumbling to self)

7. Side glances, leaning/looking forward/backward especially to look at another student’s screen

or to communicate with another student

8. Use of bonnets, bandanas, head/earphones, earbuds, airpods

**Anyone caught cheating automatically gets ZERO for the exam. This will be reported to the prefect of
discipline and the Dean’s office to disqualify you from PSWD should you fail Anatomy. This means you
will have to repeat the WHOLE year for Anatomy in case you fail.

** If any student shares the exam link and/or exam code or creates a bogus email to take the exam the
WHOLE CLASS will get ZERO.
SUBMIT ON TIME to avoid inconvenience. The link will close AUTOMATICALLY on the set time after
which, your form will NO LONGER BE ACCEPTED. NONSUBMISSION of MS form is equivalent to a grade of
ZERO.

MAKE SURE THAT YOU ARE USING YOUR OWN MS ACCOUNT DURING THE EXAM, otherwise your exam
may be considered VOID. Only your OWN MS ACCOUNT that is LOGGED IN will be considered VALID for
the exam.

The exam link will be sent to each of you individually only through this registered email. This registered
email is the only email considered VALID to login to the exam. If you have questions during the exam, call
the attention of your proctor.

PLS PRINT THIS EMAIL AND SIGN THE CONFORME BELOW.

The proctors have the prerogative to record the exam/examinees on video/photo for exam purposes. By
signing this conforme, it means that you AGREE and subject yourself to comply to the terms, conditions
and policies stated above. This form should be submitted to the proctor in your assigned room BEFORE
the start of the exam.
