FIELD REPORT
Date: January 16, 2023
Field Report: 39
Project:
CONSTRUCTION OF THE NEW PALA O PUBLIC MARKET
Subject:
NON-COMPLIANCE OF OCCUPATIONAL SAFETY AND HEALTH STANDARDS
This has reference to the ongoing implementation of the above-stated project.

Please be informed to implement the “Occupational Safety and Health Standards or OSHS” as amended by the Department
of Labor and Employment (DOLE) under “Republic Act 11058,” a “Act Strengthening Compliance with Occupational Safety
and Health Standards and Providing Penalties for Violations Thereof.” This issuance aims to protect every workingman
against the dangers of injury, sickness, or death through safe and healthful working conditions.

Below are some findings that need to comply with the minimum OSH standards.
1. No Safety Officer deployed on site.
Safety Officer must roam around and continuously inspect project sites to ensure a hazard-free environment.
2. Unsafe acts and conditions of working personnel and LGU staff. Please see "Annex A"
2.1.Workers should wear hard hats and sturdy, non-skid shoes to prevent slips and falls in working above the scaffolds
and to protect the workers below.
2.2.Each worker on a walking or working surface of 6 feet (1.83 meters) or more above a lower level must be protected
from falling by a guardrail system, a safety net system, or a personal fall arrest system. It consists of an anchorage, a body
belt, or a body harness.
2.3. Wiring that, if touched by passing workers, may result in electrical shocks and grounding and could not be left
unattended.
3. No housekeeping - Housekeeping is keeping your job site clean and tidy. Garbage must be put where it needs to be.
3.1.Remove all excreta and attractive smells of urine left by your dogs around the construction site.

Please deploy an experienced Safety Officer who knows how to ensure the safety of all employees, LGU staff, and visitors
by managing the workplace’s safety. Also, consider regular clean-up, gathering up, and removing debris to keep the work
site orderly and plan for the adequate disposal of scrap, waste, and surplus materials, especially the surroundings or outer
part of the structure. Materials should not be stockpiled in locations where they might deteriorate and potentially cause
pollution.

For your strict compliance.
