PRE- HIRED TRAINING AGREEMENT

This employment agreement hereby entered into between:

Be Healthy Grill Station a franchise company of FISH2GO with
address Anabu Kostal Market, Anabu 2D, Imus Cavite

AND

John Lorienze Petallo Costalles, Single, 18 years old with residential address
Blk 53 excess lot brgy. Sto Cristo Area 1, Dasmariñas, Cavite as the PRE-HIRED TRAINEE.

Terms and Conditions:

1. That EMPLOYEE is fully aware and acknowledges his/her status as PRE- HIRED TRAINEE
(Store Crew) for a period of 7 days, with 12 hours within the normal store hours of 7:00 am till
7:00 pm (Opening shift) and 10:00 am to 10:00 pm (Closing shift) from Monday to Sunday (No
holiday pay for PRE-HIRED TRAINEE)

2. That on the 1 week / 7 days, an evaluation/assessment will be conducted by the EMPLOYERS

duly authorized staff to perform the said function anytime of the day:
Whereas, PRE- HIRED TRAINEE should pass the evaluation / assessment. PRE-
HIRED TRAINEE shall merit the opportunity to be hired as Trainee Employee limited to the Terms and
condition set within the Employment Contract.
Whereas, if PRE-HIRED TRAINEE fails after the evaluation, EMPLOYER will notify
the PRE- HIRED TRAINEE.
3. An endorsement letter coming from the EMPLOYER`s duty authorized personnel/s will set as
Definitive in determining the official continuing training employment of PRE- HIRED
TRAINEE.
4. PRE- HIRED TRAINEE acknowledges that all implementing rules, guidelines and policies set
by the EMPLOYER shall be the considering factor to impose discipline and order.
5. PRE – HIRED TRAINEE abides and confirms that full understanding of all existing and
implementing rules and guidelines has been met and shall be in conformity in obedience under
the authority of EMPLOYER.
6. EMPLOYER reserves its right and authority to inform, warn (verbal and written), discipline,
suspend and terminate erring PRE-HIRED TRAINEE if it`s deemed necessary.
7. The PRE-HIRED TRAINEE will only be paid after signing the Probationary Contract.

The PRE-HIRED TRAINEE starting allowance will be P150 per day (15/hour) plus Ecola of
Php50.00 a total of Php 200/day. If the PRE-HIRED training is deemed terminated he will be need to pass
Clearances with authorized Signatory to receive his Pay.
If in case the PRE-HIRED TRAINEE refuses to report to work and has committed AWOL
(Absence without leave), the Company will implement a “No Payment policy” since the Company
invested time and resources just to train a non-serious/committed.

The terms of this agreement remain in effect until amended in writing and signed by the Employer. Dated
this 18th Day of August, 2019.
