Subject: ONLINE EVALUATION OF TEACHERS BY THE STUDENTS

All students are advised to evaluate the performance of their teachers for SY 2020-2021
via online. Evaluation forms are available through the individual OPIS account,
accessible via university website; www.urios.edu.ph.

The evaluation will be conducted from May 20, 2021 to June 11, 2021.

EVALUATION PROCEDURE USING THE OPIS (FOR STUDENTS)

1. Go to www.urios.edu.ph
2. Click OPISV3, choose CURRENT
3. Log-in to your OPIS V3 account - Student Access Module
4. Click Schedule
5. Click SUBJECT CODE (in color blue and underlined)
6. To answer, just click the radio button accordingly. You may change your
answer as long as you have not saved your evaluation. Only one answer per item
will be accepted.
7. Please write your COMMENTS about your teacher in the text box provided.
8. Click SAVE

Rest assured that your identity as evaluators, names and student numbers, will not
appear in the evaluation result.
