INTRODUCTION
Dedicated grade 4 teachers, leaders and school
head, lead the grade level to carry out all the
accomplishment in the second quarter. In which the result
leads to accomplish all the scheduled activities and
programs. Furthermore collaboration and unity plays an
important role in the performance of the academic and
non-academic activities with the cooperation of the
stakeholders.
Indeed Grade 4 has come to make the mission of
delivering learnings to all the learners as mirrored in this
2nd quarter accomplishment report.
Among the major accomplishments of the Grade 4 this 3rd Quarter SY 2020-
2021, are as follows:
Delivery of Educational Service:
1.Distribution and Retrieval of Self Learning Modules and
Activity Sheets
To ensure the continuous learning of the grade 4 learners despite the pandemic, Grade4
used “ CONTACTLESS DISTRIBUTION AND RETRIVAL OF SLMs”.The
parents/guardians were oriented through class group chat and School facebook Page.
Following Safety Protocols were strictly observed.

It is shown in the table that Grade 4 enrollment is composed of 123

learners under Online Distance Learning and 507 learners taking up Printed
Modular Distance Learning with a total enrolment of 630 learners under ODL
and MDL mode of learning modalities.
3.Interventions
To be “Grade Quality Learner ” is the prime goal of Grade 4 family .
In sustaining quality education in our grade , different stakeholders joined hand
in hand together to achieve our objective. This Report shows the grade
accomplishments through the exerted efforts of SDO, School Head , teachers,
parents, LGU’s, NGO’s and community members.
Notable findings based on our assessment in our grade level were
given actions to address the needs and concerns of our learners.

4.Learning Delivery
In the new normal of education setting, the classroom is
our homes and the learning space wherein the teaching-learning
processes are exchanged and the learning delivery modalities that
grade 4 offers are Online Distance Learning (ODL) and Printed
Modular Distance Learning ( MDL ) Which provides safe methods
wherein education can foster and succeed. Education, being
necessity for our learners will find its way and space in the midst of
pandemic.

5. Teacher Development
In the DepEd, a Learning Action Cell is a group of teachers
who engage in collaborative learning sessions to solve shared challenges
encountered in the school facilitated by the school head or a designated LAC
Leader. LACs will become the school-based communities of practice that are
positive, caring, and safe spaces.
7.Kamustahan
COVID-19 has brought about changes to the education system that
impact teachers in multiple ways. However, social media plays an
important role as a tool for communication for hard-to-reach students or
teachers in schools that serve hard-to-reach communities. It also the
chance or educators to check on the health status of their
learners .Among many institutions, schools have also shifted their base to
virtual platforms to conduct “Virtual Kamustahan” with pupils,
answering queries of parents/ guardians and with the support of the
school head.
8. Home Visitation
A home visit is a way to bridge the gap between school and home for students, families,
and teachers. Home visitation is a bridge for school and home in this new normal of
schooling.
Home visitation is away for the teachers and parents to work hand in hand for the
improvement of learners performance. Through AKAP PROJECT teachers find ways to
reach learners who needs assistance.
