We, _____________________ and _____________________, both of legal age,

Filipinos, both married and both residents of ___________________________, Cebu,
respectively, after having been duly sworn in accordance with law, do hereby depose
and say, T H A T:

1. We personally know XXX (X, for brevity) because we are friends and neighbors
with her parents AAA and BBB for a long time.

2. Annie was born on JAN.13, 2005 in Yati ,Liloan, Cebu. Her birth certificate was not
registered for reasons that the family had other issues at the time of her birth and
the registration was overlooked.

3. We are not in any way related to ANNIE ROSE L.SACEDA by affinity nor
consanguinity;

4. That finally, we are executing this affidavit for the purpose of informing the
authorities especially the Local Civil Registrar and the Philippine Statistics
Authority of the above-mentioned and to support the application for late registration
of birth certificate of ANNIE ROSE L. SACEDA who is the legitimate child of
MONINA CANETE LARESMA and RANNIE PEPITO SACEDA.
