2.1. Defective machine/excavator. 2.1.1. Inspected and good condition excavator should be engaged.
2.1.2. Daily operator checklist to be inspected before positioning excavator.
2.1.3. Ensure there is no oil leakage.
2.1.4.Ensure reverse horn, caution light are in working condition.
Positioning of 2.2. Unauthorised operation. 2.2.1. Only authorised operator should operate the excavator. Site Engineer

3.3. Overhead power cable/live wires. 3.3.1. Ensure safe distance from overhead power cable/live wire.
3.3.2. Work area inspection to be done before starting excavation.
3.4. Collapse of excavation edge. 3.4.1. Ensure sufficient sloping according to the nature of soil.
3.4.2. Donot stack materials, equipment on the edge of excavation, hard barrication to be done with a
distance of one and half (1.5) metre from the edge of excavation. Site Engineer

3 3.5. Overturning/ toppling of excavator. 3.5.1. Provide stop blocks for machines approaching the edge of an excavation. Supervisor

3.5.4. Only allow competent person to operate the equipment.
3.6. Fall of materials from bucket. 3.6.1. Exclude all people who are not directly involved in thework of the machine from the working area.
3.6.2. Donot overburden the bucket. Only carry the safe quantity of materials it can accommodate.
3.7. Confined Space. 3.7.1. Confined space entry permit should be followed before starting the work according to the confined
space classification mentioned in the Air products confined space entry procedure and in Technip HSE plan
3.7.2. Only engage confined space entry trained personnel for working in confined space.
4.1.Reversing vehicle with restricted rear 4.1.1. Only allow authorised inducted operators to operate the dump trucks.

4.1.4. Ensure flagmen with trucks for guidance.
4.1.5. Always use mirrors and look in direction of travel.

4 Operation of Pump 4.3. Collissions with other plant and 4.3.1. Always check mirrors and look in direction of travel. Supervisor

clearance.
4.4.4. Advise all site personnel to be constantly aware of the danger of overhead power lines.
7.1. Low visibility. 7.1.1. Low visibility hazard must be emphasized in tool box meeting.
7.1.2. Ensure sufficient site lighting for job where required.
7.1.3. Red/green light batons to be used for guiding vehicles during night works.
7.2. Fatigue 7.2.1. Adhere to good fatigue management practices. Site Engineer
5 Night work
7.2.2. Get a good session of sleep prior to commencing shift. Supervisor
7.2.3. Report to foremen/supervisor when symptoms of fatigue affect safe operation. Foreman
8.1. Dehydration. 8.1.1. Ensure sufficient drinking water provided at work area/rest shelter.
8.1.2. Operatives are instructed to take regular breaks as per the client heat stress index.
