Dear Mam,

We would like to request for the approval of the demolition of a warehouse located at ______________This
is in relation with our construction company's on-going construction of Slope Protection with an approved
budget contract amounting to PhP 154,099.79. The above-mentioned warehouse is situated at the actual
site of the slope protection being constructed. We cannot proceed to the construction of the project
because we were not allowed to demolish it and advised us by the PREC Center Chief to address this
issue through a formal letter request.

Moreover, we would also like to seek your good office technical assistance to locate the actual site of
another project which is the access road amounting to PhP 32,975.00 which is located at the _________-.

We hope for your prompt action regarding on this matter since the Slope Protection project is almost 100%
accomplished.

Attach here with this photocopy of the said warehouse to be demolish
