Chapter 1: Dimensions of Wellness & Behaviour Change

How healthy are you in each of the 7 dimensions of wellness?

Dimension Rating 1 - 10 Why did you give yourself that rating?

Social
7 I migrated to Canada several years ago
and although I am living with my family, I
still miss my friends back in the Philippines.

Occupational
9 Many opportunities make me go. I worked
as a support worker for four years with a
lovely family, and I cannot thank them
enough for how there are friendly to me
and supportive they are me in pursuing my
nursing career.

Spiritual
8 I am not as religious as I can be, but I talk
to Him quietly. Continuation to life as
reincarnation and karma is my spiritual
philosophy.

Physical
6 Physically, I am decent. Not too healthy,
and not too unhealthy too. I rate myself six
because I eat unhealthy foods that keep
me going as a travel care aide, and at the
same time, walking with my dog when I
am home makes me feel guilty about being
unhealthy.

Intellectual
7 I love being challenged in most aspects of
life because it stretches our minds to keep
more active, yet I also doubt myself. Too
busy to think about myself sometimes.

Emotional
7 I rate myself seven for a reason of being
overwhelmed by being a student and
working at the same time. I am strongly
positive and motivated in everything I do. I
occasionally treat myself to a massage to
stay positive and keep me going.
Environmental
8 I tried to help making our environment
clean and green by segregating garbage at
home and use organic materials and food
as much as possible.

you change a behaviour in order to improve one of the dimensions?

nursing diploma within next
two years and
start working in hospital.

3. I will treat myself to yoga twice a month starting next week.
