Aliza: hello, Ms. Dela Cerna

Me: hello Good morning nice to meet you

Aliza: Nice to meet you too I’m aliza bakil, I’m a student nurse from ateneo
de zamboanga university and im the one who is assign for you for the
following check up

Me: please come on in

Aliza: thank you ma’am

Me: no its okay I won’t mind, let go over the living room

Aliza: may I place my bag on the top of the table

Me: yeah sure, it’s okay

(Place the bag on the table lined with a clean paper or drape because I don’t
want to bring anything into your home and also to protect the bag from
getting contaminated)

(Proper hygiene)

(Open the bag and take out the towel and soap, wash hands using soap with
water and dry it using the towel) (If walang tubig sa faucet mag ask ng basin
or gamitin ang alcohol or hand sanitizer for proper hand hygiene)

(take out the apron from the bag then put it on with right side out to protect
the nurse uniform)

(Put out all the necessary articles needed for the specific care)

(Close the bag and put it on in one corner of the working area)

Aliza: I will give you a consent form sign and get some basic information
from you.

Aliza: so I’m giving you the consent form and could you sign it for me

Me: ohhh okay

Aliza: next is the family identification information I prefilled out some of the
information already if you could just double check if it is correct
Me: yes ma’am, it is correct

Aliza: May I know your birthdate Ms. Gwy?

Me: okay, so it’s February 25, 2001

Aliza: any one is in the house?

Me: my cousin, Ms. Althea serrador

Aliza: and her birthday?

Me: October 29, 2005

(Proceed on performing the necessary nursing care and treatment)

Aliza: alright, so im going to prepare your medicine

When preparing liquid medication:

 Thoroughly mix the medication before pouring


 Remove the cap and place it down on the countertop
 Hold the bottle and place the medication cup at eye level and fill it to
the desired level
 Before closing or capping the bottle, wipe the lid or the top of the
medication with paper towel

Explain the purpose of the medication and how it will help using the
language that the client understand.

 Administer the medication at the correct time


 Give the client sufficient water to swallow the medication
 Stay with the client until all medication have been swallowed
 Document each medication given
 Dispose all supplies appropriately
 Perform hand hygiene

Evaluate the effects of the medication

Aliza: do you take any medication aside from the medication that you are
taking right now?

Me: it’s just the same medication that the one you gave me

Aliza: oh okay, how often do you take your med

Me: every 4 hours or after I eat my meals

Aliza: how was that been working for you

Me: I think it’s pretty good because it lessen the itchiness in my throat and I
can sleep well and medyo gumaan na yung pakiramdam ko hindi na tulad
nung una

Aliza: so since I’m here doing this visits, what you feel like you to work on
how to stop your cough

Me: well, I will not eat any food that makes my throat itchy and also I will not
drink any cold drinks and most importantly I will not skip my medication

Aliza: well that a great way. You already know what to do. I think our last
visit is on September 1 which is Wednesday. So it’s been really nice meeting
you I look forward to working with you for the next few weeks. Thank you

Me: it’s also nice meeting you, thank you for coming.

(Open the bag and return all things that were used and perform hand
washing)

(Remove apron, folding it away from the person, the soiled side in and the
clean side out. Place it in the bag)

(Take and record and have a talk with the mother. Write down all the
necessary data that were gathered, observed, nursing care and treatment
rendered)

(Make appointment for the next visit (either home or clinic) taking note of the
date and time)
