Case:
Since last night, mrs. Afifah complained of shortness of breath. Mrs. Afifah has a history of
asthma since 2 years ago. And last night mrs. Afifah’s asthma relapsed. And she made an
appointment with the doctor rasyid at the clinic.
Making Appointment
Afifah: “Good morning, I would like to make an appointment with Dr. Rasyid.”
Arneta: “Alright ma'am, where do you live? And can I know your phone number?”
Arneta: “Alright ma'am when will you make an appointment with Dr. Rasyid?”
Arneta: “Okay ma'am, is there anything else I can help you with?”
Afifah : “afifah”
Afifah : “0203”
Arneta : “From the data that I found, Mrs. Afifah had an appointment for a health
consultation at 8 am, right?”
Arneta: Last night my mother complained of being cold, suddenly my mother was short of
breath and coughed.
Rasyid: Alright, so the cold air makes the lungs contract. As a result, the airways can tighten
and trigger your mother's asthma to recur. Ma'am, here I will do a chest auscultation check to
find out the sound of your breath.
Rasyid: Okay, let's start, ok. Later when I put the stethoscope on your chest, please take a
deep breath.
Rasyid: Okay, I will count one to three, you take a deep breath.
Rasyid: Alright ma'am, after I did the examination on your chest, it turned out that there is a
weezing sound. This is caused by narrowing of the airways. So that's what causes shortness
of breath.
Rasyid: It is better if your mother is hospitalized so we can monitor your mother's condition.
And the nurse will do some tests to find out what treatment give to your mother.
Arneta: Ok doctor.
Rasyid: If you don't have any question, you can book an in patient room in advance.
Alifah: My name is Alifah. Here I will check oxygen levels in your blood using an oximeter,
are you willing to do it?
Alifah: The results of the examination show that your blood oxygen levels are low with a
value of 92%. While normal oxygen levels in the blood are 95-100%. There fore it is
necessary to do oxygenation therapy. Later, there will be other nurses who will help your to
carry out oxygenation therapy.
Afifah: Alright.
Afifah: No thanks.
Anne : May I see the identification bracelest first? Yes, it is true. Well, I am Anne, nurse on
duty from from 7 am to 2 pm. How do you feel?
Anne : Well, today we are going to do oxygen therapy to reduce your shortness of breath, it
takes around 10 minutes. Before that, what do you want to ask?
Afifah : Nothing.
Anne : Well, I am going to close the curtains to protect your privacy. Okay Ma’am, I will
start the action but before that, I will prepare the tools first.
Afifah : Okay.
Anne : Well Ma’am, I’m sorry, it maybe a little bit uncomfortable but it is to help your
breathing, I will give you 2½ liters of oxygen.
Anne : Well, I will be right back at 2 pm to check your nasal cannula and nasal mucosa
whether it is dry or not, later you will be given further treatment if the nasal mucosa is
dry. Before I leave this room, do you have any question Ma’am?
Afifah : Nothing.
Anne : Well, if you need my help you can press the button on the right side of bed or ask
your family to call me.
At 2 pm the nurse back to the Mrs. Afifah’s room to absorb the nasal cannula and nasal
mucosa
Afifah : Of course.
Anne : Well, as we promised I will check your nasal cannula and nasal mucosa that is
installed on your nose, the goal is to find out whether further treatment is needed if
your nasal mucosa is dry.
Anne : Okay Ma’am, I will be back to nurse’s room, before that, what do you want to ask?
Afifah : Nothing.
after 2 days of therapy or medication, the patient's condition improved and was allowed
to go home. (Nurse 1)
