총점이 높을수록 우울증상의 정도가 심함을 의미한다. 본 연구에서는 선행연구(Irwin et al., 1999)를
참고로 하여 원자료의 0점과 1점에 해당하는 경우를 ‘증상 없음’으로, 2점과 3점에 해당하는 경우
를 ‘증상 있음’으로 변환하여 분석하였으며, 가능한 점수의 범위 0∼10점 중 4점 이상일 경우 임상
적으로 의미 있는 우울증상이 있는 것으로 간주하였다(Table 2).

3.3. 일반적 특성, 건강관련 특성, 우울, ADL, IADL 수준에 따른 독거노인과 비독거노인의 인
지기능 저하의 차이
모든 변수가 인지기능 저하에 유의한 영향을 미쳤다. 이에 다중 로지스틱 회귀분석 결과, 독거노인
집단에서 인지기능 저하에 영향을 미치는 요인은 저학력, IADL 제한이 있는 경우, 친구를 만나는
횟수가 적은 경우, 고령, 규칙적으로 신체운동을 하지 않는 경우 등이었다. 즉 초등학교 졸업 이하
의 저학력인 경우 인지기능 저하 위험이 약 2.9배 높았고(OR=2.908; 95% CI=1.052∼8.036), IADL
제한이 있는 경우 2.636배(95% CI=1.249∼5.564), 친구를 1년에 1회 정도 만날 경우 2.569배(95%
CI=1.218∼5.417), 75세 이상의 고령인 경우 2.048배(95% CI=1.440∼2.913), 규칙적인 신체활동을 하
지 않는 경우 1.535배(95% CI=1.045∼2.255)로 높았다(Table 4).

비독거노인 집단에서 인지기능 저하에 영향을 미치는 요인은 친구를 만나는 횟수가 적은 경우,
ADL 제한이 있는 경우, 고령, 저학력, 주관적 건강상태가 나쁜 경우, IADL 제한이 있는 경우, 여
성인 경우 등이었다. 즉 친구 만남 횟수가 거의 없는 경우 약 3.5배의 인지기능 저하위험을 보였으
며(OR=3.515; 95% CI=1.053∼11.730), ADL 제한이 있는 경우 3.297배(95% CI=1.820∼5.974), 75세
이상의 고령인 경우 2.477배(95% CI=2.052∼2.992), 초등학교 졸업 이하의 저학력인 경우 2.573배
(95% CI=1.685∼3.927), 주관적 건강상태가 나쁜 편인 경우 1.82배(95% CI=1.372∼2.414), IADL 제
한이 있는 경우 1.907배(95% CI=1.422∼2.558), 여성인 경우 1.59배(95% CI=1.237∼2.043)의 인지기
능 저하 위험이 있음이 나타났다(Table 5).
4. 논의
본 연구에서는 독거노인과 비독거노인 집단의 인지기능 저하에 영향을 미치는 요인을 파악하고

5. 결론 및 제언
본 연구의 결과, 독거노인 집단과 비독거노인 집단의 인지기능 저하에 공통적으로 유의한 영향
을 미치는 요인은 고령, 저학력, IADL에 제한이 있는 경우와 적은 친구만남 횟수 등이었다. 일반
적 특성을 보정했을 때 독거노인 집단에서만 나타난 영향요인은 규칙적으로 신체운동을 하지 않는
경우 등이었던 반면, 비독거노인 집단에서만 나타난 영향요인은 ADL 제한이 있는 경우, 우울증이
있는 경우, 낮은 주관적 건강상태 등이었다. 이렇게 독거여부에 따라 다르게 나타나는 영향요인들
을 고려하여 집단별로 보다 효과적인 인지기능 유지 또는 인지기능 저하의 지연을 위한 개별적 중
재방안이 필요한 것으로 보인다. 또한 두 집단에 대한 장기적인 추적관찰을 통하여 인지기능 저하
에 대한 영향요인이 어떠한 패턴으로 변화해 가는지에 대한 연구를 바탕으로 맞춤형 간호 및 치료
중재를 개발 및 제공함으로써 보다 효율적으로 인지기능 저하를 지연시키고 치매 예방 전략을 모
색하는 것이 필요하다고 사료된다.

Abstract
This study aimed to identify the factors influencing cognitive impairment in
community-dwelling older adults living alone and those living with others using the 4th
basic survey of the Korean longitudinal study of ageing (KLoSA) 2012. The study sample
consisted of 4,134 persons aged 65 years old and over who participated in the KLoSA
2012. Measurements included the K-MMSE for cognitive function, the CES-D 10-item scale
for depression, the K-ADL and the K-IADL. Logistic regression analyses were conducted to
identify factors influencing cognitive impairment in two groups. The common factors that
significantly influenced cognitive impairment in both groups were old age, low education
level, limited IADL, and infrequent visiting with friends. When adjusted for demographic
characteristics, not performing regular exercise were significant factors found only in the
living-alone group whereas limited ADL, the presence of depression, and low subjective
health status were the significant factors found only in those living with others. Further
studies need to be conducted regarding development of tailored nursing and treatment
intervention strategies to prevent or delay cognitive impairment for these two groups.
