Interested suppliers are required to submit their valid and current Mayor’s permit,
Income/Business Tax Return (for ABCs above PhP500k), Philgeps Registration Number,
Omnibus Sworn Statement, upon submission of quotation. PhilGEPS Platinum Membership
may be submitted in lieu of the Mayor’s/ Business Permit and the PhilGEPS Registration Number.

Should you wish to submit your bids online, you may send the same to 109740@deped.gov.ph
Kindly take note that the same shall be encrypted with password. Such bid will only be
decrypted during the opening of RFQ. If the submitted online documents are not password
protected it would not open during the Opening of RFQ, then it shall be Disqualified (DQ).

Award of contract shall be made to the lowest quotation, which complies with the minimum
description as stated above and other terms and conditions stated in the price quotation form.

Any interlineations, erasures or overwriting shall be valid only if there are signed or initialed by
the bidder or his/her duly authorized representative/s.

Submission of quotation and eligibility documents is on or before 2:30 p.m. of August 30, 2023
at Isabang Elementary School Principals Office, Barangay Isabang Lucena City. Please
submit your quotation in a sealed envelope addressed to:
