Should the task(s) scope of work changes or the conditions on site, or the circumstances under which the risk assessment was carried out change significantly or if established control measures
fail due to unforeseen circumstances e.g. Unsecured dropped object which comes into contact with personnel; unauthorized personnel entering established barriers during critical work;
extreme weather conditions etc.
Employees performing those tasks should request that this assessment be stopped and reviewed to reflect those task changes and identify the associated risks
