STUDENT’S PLEDGE AND WAIVER

That I promise to obey and abide by all rules and regulations promulgated and enforced by the
BISU Balilihan Campus, its instructors and other constituted authorities.

1. That the BISU Balilihan Campus shall not be held liable for any accident, harm or injury that may
happen to me while in the performance of my duties and assignments as a student the cause
which is due to failure to heed warnings, cautions, signals and/or my neglect to obey by the
rules and regulations prescribed and promulgated by the school authorities and I hereby further
waive and renounce my right to claim for damages against the BISU for any accident, harm or
injury which may happen to me in the manner and time specified.

2. That I have read and fully understood the contents of this pledge and waiver, and I affix my
signature herein freely and voluntarily.
