1. Monitoring Well Label
No label.
Verify monitoring well label to confirm it has a unique
identification. This is to avoid incorrectly reported test results. X

2. Monitoring Well location and accessibility.

Check the physical location of the well for obstructions when

conducting visual inspection. P
3. Locking Devise
The locking system of the well is in good condition. X No Lock
4. Protective Cover
Ensure a permanent steel protective cover is installed. A steel Cover already corroded
protective cover with locking cap is needed to secure and protect

the well from damage and unnecessary intrusions.

5. Well Casing
Check if the well casing moved, sunk or was bent from its original
construction position that may compromise accurate water level
measurement or the sampling procedure.

Ensure presence of established reference water level measuring No water level marker observed.

Check for presence and condition of the inner well cap vent. A Cap installed but not vented
type.
