AFFIDAVIT OF LOSS

I, MIGUEL FAUSTINO G. ALONZO, of legal age, single, Filipino,

and a resident of Poblacion Zone 4, Baybay City, Leyte, after having
been duly sworn to in accordance with law, hereby depose and say;

1. That I am a holder of a School Identification (ID) Card issued by

Saint Paul School of Professional Studies, Palo, Leyte;

2. That however, said School ID Card inadvertently fell without my

knowledge when I was at a public utility vehicle (PUV) last
September 23, 2023, and the fact of said loss was only
discovered on September 24, 2023 when I was already looking
for it;

3. That despite diligent effort to search, save, and retrieve the

aforementioned School ID Card, my effort proved to be futile and
is now believed to be lost beyond recovery;

4. The aforementioned School ID Card was not confiscated by any

government entity or authority for any violation of the Philippine
law;

5. That I am executing this affidavit to attest to the fact and

truthfulness of the foregoing statements and for whatever legal
purposes this may serve.
