AFFIDAVIT
(CONSENT)

I, SHERYN G. VARGAS, of legal age, married, Filipino, and a

resident of Brgy. Buenavista, Baybay City, Leyte, after having been
duly sworn in accordance with law, do hereby depose and say:

1. That I am the mother of ROHANNEE G. VARGAS, 17 years

old who was born on June 10, 2006 at Tacloban City;

2. My daughter is applying for student permit to drive with the

Land Transport Office (LTO), Tacloban City;

3. One of the requirements is my consent and that by virtue of

this instrument, I am hereby giving my consent to my minor
daughter ROHANNEE G. VARGAS to apply for a Student
Driver’s License with the Land Transportation Office (LTO),
Tacloban City;

4. I am fully aware of the consequences, effect and

responsibilities on my giving of consent;

5. That I am executing this affidavit to attest to the truth and

for whatever legal purpose it may serve.
