I am SR. MARIA RIZMA M. BARZO, FMA and I do hereby authorize Vicente

Ricaplaza or Victoria Mendez, our school Security Guard, to claim my document
package from 2GO Express. I am authorizing them to act on my behalf as I won’t
be able to personally claim it due to bad health conditions.

I have appended two of my IDs – school ID and SSS ID – to serve as pieces of

proof that I am allowing the bearer to do the task for me.
