AFFIDAVIT OF RECTIFICATION OF GRADES

I, NERISSA TEVES SARMIENTO, of legal age, Filipino Citizen, and with postal and
residence address at Block 15 Lot 1 Tindalo St. Villa Luisa Homes, San Agustin I, Dasmarinas
City, Cavite, after having been duly sworn to in accordance with law, hereby depose and say:

That I am a clinical instructor at Emilio Aguinaldo College-Dasmarinas City, Cavite,

handling students on Care of Mother, Child and Adolescent (Lecture) under sections
BSN 2-8;

That unfortunately, I have inadvertently entered incorrect grades for my students,

particularly Ms. Maria Shariza Carlos Bagtas and Ms. Ira Nicolle Bron Morales for the
Prelims;

That through this affidavit, I would like to correct and change their grades into the
following:

 Ms. Maria Shariza Carlos Bagtas – from 76.85 to INC

 Ms. Ira Nicolle Bron Morales – from INC to 76.03

That I already recomputed their grades and made a spreadsheet of their revised grades;
the updated class records is hereunto attached as annex “A”;

That the said erroneous was not intended or act of fraudulent to commit offence against
the school provisions;

That I am executing this affidavit to attest to the truth of the foregoing facts for all legal
intents and purposes.
