We, MARICAR C. MORENO and CHRISTIAN KENT C. CASAS, both of legal ages, single,
Filipino and resident of La Carlota City, Philippines, after having been duly sworn to in
accordance with law, do herby depose and say.
That we personally know BILLY FACULTAD NOVILLA of La Carlota City, Negros
Occidental.
That in his marriage contract his name is recorded as OLIGARIO (BILLY) NOVILLA, which
erroneous and not correct, instead of his true and correct name as BILLY FACULTAN NOVILLA;
That the name OLIGARIO (BILLY) NOVILLA and BILLY FACULTAD NOVILLA just refer to
one and the same person;
That we are aware of the ____ fact because we are close friends and acquaintance of
the subject person;
That we are not related to the subject person by a ____ or consanguinity, ____ do we
have any primary interest in the _____ of this affidavit.
