Initial Information
The child was observed during the session by the trainee school counselor. The
trainee school counselor interacted with the child through greeting and usual conversation
about her likes and dislikes. F.M. was a girl of 15 years old who studied in class 8th A.
Apparently, the client seemed to be well-kept. But from her physique, she looked younger
than her chronological age. As she seemed to be thin and frail. She wore the proper, neat, and
clean uniform with a scarf. Her shoes were polished, her nails were trimmed and her hair was
combed properly. The complexion of the client was dull and pale. The client’s hygiene was
well maintained. The child’s facial expression was composed during the session. Sometimes
she gave a reserved expression and other times she had a smile on her face. In the very first
session, the trainee came into the client’s classroom and asked her teacher to bring her to the
session. The client came with the trainee school counselor with smiley facial expressions.
The child's eye contact was appropriate but sometimes used to lower her gaze. Her sitting
posture was bent a little and her body movement was appropriate. The child was showing
compliance toward the trainee school counselor. The child used to communicate with low
pitch, but whenever after asked again something child used to increase little bit his voice. Her
tone of voice was not clear as she had difficulty in producing some sounds. The child had
difficulty listening to the voice of the trainee school counselor as the trainee had to repeat her
statements many times in session. The child mentioned that she had a hearing problem from
birth. And now she was wearing a hearing device. She reported that after her birth the doctor
kept her in an incubator for 7 days and discharged her mother. Further information was not
provided by the child in this regard as she mentioned that she didn’t know any other
information. Due to the hearing problem, she had difficulty in her studies and she was unable
to read properly she was also facing a problem remembering her lesson and combining
syllables to produce a word.
Initial Assessment
Assessment modalities included.

● Behavioral Observation

● Clinical interview of the child

● Subjective rating of the problem

Preliminary Findings
The preliminary findings emphasize the profound impact of hearing impairment on
child’s academic performance and overall well-being. Her moderate hearing loss has created
significant barriers to learning. Child’s academic performance was assessed through CBA.
She demonstrated significant difficulties in language-based tasks, particularly in reading, and
comprehension. Child’s scores were below grade level in most academic areas, suggesting a
considerable impact of her hearing impairment on learning.
Provisional Formulation
On the basis of available information, it was hypothesized that there are some factors
that contributed in clients current condition i.e. postnatal complications, hearing problem,
studying in mainstream school, ineffective teaching methodology, and problem in arithmetic
skills. According to information available, it seemed that client’s complications after birth
cause her hearing impairment and parental negligence maintained her problem. Furthermore,
ineffective teaching methodology create hinderance in her academics.
