Sir,
I hope this letter finds you in good health and high spirits. I am writing to bring to
your attention several discrepancies and inaccuracies in the registration of birth for
MRS. BELLA AGPAOA VIERNES, and I kindly request your assistance in rectifying
these issues. The birth registration details are as follows:
1. Date of Birth: The birth certificate of Mrs. Bella states her date of birth (also in
her negative certificate) as August 8, 1940. However, the majority of supporting
documents, including her Tax Declaration, Barangay Certificate, Senior Citizen's
ID consistently indicate her date of birth as August 3, 1943. Additionally, there is
another document (voter’s certificate) that suggests her date of birth as August 3,
1942. We kindly request your office to investigate and correct this discrepancy to
reflect the accurate date of birth.
2. Place of Birth: There is no evidence of the place of birth. To address this
omission, we kindly request that the place of birth be added based on the
available information or supporting documents.
3. Spelling of Middle Name of Mother: The middle name of her mother "Agcawili"
as entered in her COLB differs from the usual spelling, which is commonly
spelled as "Agcaoili." This inconsistency in spelling may lead to confusion.
4. Similar First Names: It has come to our attention that the first names of Mrs.
Bella's mother and father are the same. We believe this is an oversight and
request that their correct first names be registered.

We look forward to your response and the subsequent corrections to Mrs. Bella’s birth
certificate.
