AUTHORIZATION TO APPLY FOR AUTHORITY TO PRINT WITH

THE BUREAU OF INTERNAL REVENUE

WHEREAS, (COMPANY) is a non-stock, non-profit Association duly registered with the

Securities and Exchange Commission with principal place of business located at (ADDRESS).
WHEREAS, as a duly registered taxpayer, (COMPANY) issues pre-numbered and duly
registered official receipts, acknowledgment receipts, billing statements, entry tickets and the
like (the “Accountable Forms”) for its regular transactions in accordance with the rules and
regulations of the Bureau of Internal Revenue (BIR);
WHEREAS, there is an urgent need to replenish one or some of the Accountable Forms due to
the volume of transactions, and in this regard, to apply for a BIR Authority to Print (ATP);
WHEREAS, in a meeting by the Board of Directors of the (COMPANY) at the (VENUE) held
on (DATE), it was agreed and approved unanimously by the Board of Governors to authorize the
application for a BIR ATP and for this purpose, to designate its authorized representative(s);
RESOLVED, THEREFORE, that (COMPANY) be authorized to apply for a BIR ATP
Accountable Forms including but not limited to official receipts, statements of accounts,
provisional receipts, entry trip tickets, and such other accountable forms as are prescribed and
necessary for its regular commercial transactions and for this purpose, to authorize the following
representatives to sign and submit such application forms, letters and other documents and to
receive the duly approved BIR ATP Permit and pre-numbered official receipts and other
accountable forms for and on behalf of (COMPANY):

RESOLVED, FURTHER, that the foregoing authorized representatives are likewise authorized
to deputize (PRINTER) to submit the BIR ATP application and to secure the BIR’s approval for
this purpose.”
