    Dear Sir,

    With due respect, I would like to state that I am Md. Ismail serving in your Organization
    as Driver (Microbus). This is to inform you that my wife was burn on fire on 12 th
    February, 2011 while cooking in the kitchen at the night. She was severely burned and
    hospitalized for the uncertain fire burn and admitted at 12 th February for 2 days and
    released 14th February as per Doctor’s concern. I also got fired and injured in this
    accident. In this situation I am requesting for a Medical assistance.

    May I therefore hope and pray that you are so kind to consider my case with great
    sympathy and grand my application.
