Min 1/10 Patient Safety

Aseptic technique should be observed at all
times while doing the procedure.
The director highlighted that IV fluid bottles
and blood bags are left hanging after
administering fluids.
Instruments are left soaked and not cleaned
after the procedure.
Observe proper hand washing :
● Before touching patient
● After a procedure
● After encounter with body fluid
● Touching surrounding

Min 2/10 Duty allocation

The director highlighted that all staff should
attend all duties at all times.
All staff must work for 40 hours to increase
work output.
Local arrangement should be reported early to
the office.
Min 3/10 Admissions
All patients should be seen by the clinical
officer/ doctor on call during night duty.
Always speak up for our patients

Min 4/ 20 Hospital phone

No calling the patient's relatives .
Phones should be for official use only.
One should always listen and be polite while
handling the patient.
Establish good rapport with the patient.

Min 5/ 10 Health education

One should not backbite a patient even when
they are unconscious.
Discuss about the patient away from them.
Always explain the procedure to the patient.

Min 6/ 10 Time management

Reporting time - End of shift
MCH 8:00am - 5:00pm
OPD 8:00am - 5:00pm
WARD 7:30-8:00am - 6:30pm
NIGHT 6;30pm - 7:30pm
DUTY
Minimal use of phones.
Avoid conflict at all times.
Always explain to patients while handing over
shifts.

Min 7/ 10 Duty Allocation

The persons incharge should allocate the
duties and the staff should work as a team.
All the ill patients should have a nursing care
plan.
Prescribing and ordering of drugs on Medboss
should be done only by a clinician.

Min 8/ 10
Resuscitation tray should be restored and
checked every week for expired drugs.
Performance appraisal has not been charged,
the staff should set their targets.
All staff should work within their practice
licence and should renew their licence on time.
All staff should fill the welfare forms.
